Hi folks... great extension, but having a few issues (most of which I can overcome myself)
I need to put an image into the contact form in the red outlined area on the attached JPG, but cant see how to do it. Can you tell me how to do this?
Also..... is there an option for Radio Button field types in addition to Checkboxes?

0The only way I can think is to put an image in a custom HTML module, publish that module in the module position on the right side of the contact form and then through the CSS move the image to the correct position. Another way is to add some custom css code which will add a background image to the complete form
There is no option for Radio buttons I am afraid
